Demi Lovato is opening up about the problems that led her to seek treatment at a rehab facility. The 'Sonny With a Chance' star says she battled an eating disorder and depression, in addition to admitting that she was cutting herself.

"I basically had a nervous breakdown. I was really bad off," Demi tells Seventeen Magazine. "My parents and my manager pulled me aside and said, 'You need to get some help.' It was an intervention. I wanted freedom from the inner demons. I wanted to start my life over."

Demi's problems boiled over when she reportedly punched one of her backup dancers in the face. "Lashing out was my cry for help," she explains to Seventeen. "Wow, who was I to do that? I'm very ashamed. I wasn't in the right state of mind at all."

Now, Demi is teaming up with Seventeen and the Jed Foundation for the Love Is Louder Than the Pressure to Be Perfect campaign. "I'm so excited to be working with Seventeen and the Jed Foundation to help other teens talk about and cope with the problems we all face," remarks Demi. "My fans have been such an incredible support system for me -- I hope I can repay this and help other girls learn from my experiences."

As part of the campaign, the actress-singer will serve as a Contributing Editor to Seventeen by writing posts twice a month on the magazine's website.

Demi is also set to discuss her personal issues on the April 22 edition of ABC's '20/20.'
