Demi Lovato delivers a personal message of thanks to her fans in a new video  -- which discusses her return to work following a stint in rehab.

The 80-second video shows a beaming Lovato sitting in front of her fireplace, expressing her appreciation: "I am back and I am home. But I wouldn't be here today if it wasn't for the support that you guys have given me along the way. I read all of the tweets, the trending topics, the fan mail, and your support is what got me through this."

"I can't tell you how much light you guys brought into my life in probably the darkest time of my life," she adds.

The 18-year-old actress and pop singer dropped off her tour with the Jonas Brothers in November to check into rehab, reportedly for self-esteem issues that included an eating disorder and cutting herself.

Though never specifying why she went to rehab, Lovato says she hopes to someday raise public awareness of "issues that girls of all ages are dealing with."

Lovato is the star of Disney's 'Sonny With a Chance' and has released two solo studio albums, the last of which, 2009's 'Here We Go Again,' debuted at No. 1 on the album chart.
